Registered Practical Nurse (RPN) (Contract)

This role delivers community-based consultation and treatment services as part of the Behavioural Supports Ontario Outreach Team to improve the quality of life for older adults with responsive behaviors and their care partners. The position involves providing direct and indirect care, conducting nursing assessments, and developing goal-directed care plans within a rural setting. The role requires collaboration with various stakeholders, including geriatric psychiatrists and community agencies, to integrate best practices in geriatric mental health and behavioral health.
