Rehabilitation Specialist

The Rehabilitation Specialist supports adults with severe and persistent mental illness in their recovery process by helping them restore competencies and achieve success in major life domains such as housing, employment, and relationships. The role involves providing trauma-informed care, conducting assessments, developing individualized treatment plans, and coordinating access to psychiatric, medical, and social services. The specialist works within a community-based Full Service Partnership (FSP) program to promote wellness and independence for clients.
