Remote Care Navigator – Cardiac

A full-time, 100% remote Care Navigator supports a growing population of medically complex cardiac patients, primarily those with congestive heart failure, through telephonic outreach and care plan coordination. This non-clinical role focuses on patient education, Transitional Care Management follow-ups, and CMS-compliant documentation to reduce avoidable hospitalizations. The position requires working under the supervision of RN Care Managers and utilizing AI tools and care management software to efficiently manage patient caseloads.
