This role combines core remote prescribing responsibilities with the delivery of the North West London Cardio-Renal Metabolic Enhanced Service, offering a varied caseload split between general remote consultations and structured long-term condition care planning. The position involves conducting remote consultations, medication reviews, and CRM-specific appointments to optimise patient care for cardiovascular, renal, and metabolic conditions. The postholder will collaborate with multidisciplinary teams to ensure accurate clinical records and contribute to quality improvement initiatives.
