Remote Telephonic RN Case Manager–Special Needs Plan - Remote (California RN License Required) (Bilingual preferred)

Alignment Health is seeking a compassionate Registered Nurse to serve as a Telephonic RN Case Manager for Special Needs Plan members. This fully remote role involves providing telephonic case management to medically complex and chronically ill seniors, conducting health assessments, and coordinating care with various partners. The position requires an active California RN license and at least two years of clinical nursing experience.
