The Codecordinator supports Hierarchical Condition Category (HCC) coding risk adjustment initiatives by preparing medical records, reviewing documentation for accurate diagnosis code capture, and educating providers. This role partners with clinical staff, coding teams, and operational leadership to optimize HCC capture and improve documentation integrity across value-based care contracts.

Responsibilities

Prepare and manage risk adjustment visit workflows and patient lists
Conduct comprehensive pre-visit chart reviews to validate ICD-10-CM diagnoses
Perform post-visit documentation analysis for proper code assignment
Deliver ongoing education and feedback to providers on documentation standards
Conduct routine chart audits to assess documentation quality and coding accuracy
