About this role
The Care Manager partners with patients, families, caregivers and providers to deliver organized, coordinated, cost-effective, patient-centered care. They apply evidence-based interventions across populations to promote disease prevention and management and to improve outcomes such as patient satisfaction and reduced hospital and ED utilization. The role collaborates with interdisciplinary staff and stakeholders across the care continuum to support optimal health.
