About this role
The Case Manager/Registered Nurse (RN) works within the HCBA Medi-Cal Waiver program to support older adults and adults with disabilities to remain in community-based residences. The role performs comprehensive nursing health assessments, develops Plans of Treatment, and coordinates Waiver and State Plan services with multidisciplinary teams. This position is based near Palm Springs and requires travel throughout Riverside and San Bernardino counties.
