PACE North
PACE North is seeking an RN Care Manager to serve as a decision maker, advocate, and trusted partner in care for adults 55+ who qualify for nursing facility care. The role involves collaborating with an interdisciplinary team to shape care plans, conduct health assessments, and coordinate supplies and services to help participants remain independent in their homes. This full-time, in-person position requires strong clinical judgment and a compassionate approach to supporting older adults and their families.
