RN-Care Navigator

The RN-Care Navigator provides consistent care coordination across the continuum, assessing the physical, psychological, and social needs of patients while serving as an ongoing resource. This role involves organizing clinical visit needs, providing education and support to patients and families, and facilitating case conferences to ensure compliance with care plans. The position also focuses on creating and monitoring clinical quality indicators to support program evaluation and regulatory requirements.
