About this role
Ennoble Care is seeking a full-time Registered Nurse Case Manager to provide in-home hospice nursing for patients in Atlantic and Cumberland Counties, NJ. The RN Case Manager will work as part of an interdisciplinary team to support patient comfort and health, coordinate care with physicians and other providers, and engage with patients' loved ones and caregivers. The role supports Ennoble Care’s mobile hospice services and requires travel to patient homes.
