RN Case Manager- Beaumont, TX

The RN Case Manager at DaVita Integrated Kidney Care coordinates and facilitates integrated care for patients with end-stage renal disease (ESRD) and chronic kidney disease (CKD), working to improve outcomes and reduce total cost of care. The role is field-based and hybrid, supporting patients across an assigned territory while collaborating with physicians, care teams, families, and community resources. It contributes to DaVita’s integrated care model and organizational vision to make coordinated kidney care the standard of care.
