RN Case Manager Experienced (IKC)

The RN Case Manager at DaVita Integrated Kidney Care provides integrated care coordination for patients with late-stage chronic kidney disease (CKD) and end stage renal disease (ESRD). This hybrid, full-time role is primarily remote but requires regular in-person patient visits within an assigned territory and supports DaVita’s mission to improve patient outcomes and reduce total cost of care. The position works collaboratively with interdisciplinary teams to support quality, cost-effective care.
