RN Case Manager for Hospice

This role involves administering skilled nursing care to patients on an intermittent basis in their place of residence, following physician orders and a plan of care under the Clinical Manager's supervision. The position requires conducting initial evaluations, assessing patient eligibility and homebound status, and developing comprehensive care plans in coordination with the interdisciplinary team. The RN will regularly reevaluate patient needs, coordinate services with physicians and other care providers, and provide counseling and education to patients and their families.
