About this role
The RN Case Manager coordinates patient care from admission through discharge, developing comprehensive case management plans to ensure safe transitions and appropriate use of resources. The position focuses on managing patients’ progression of care and facilitating timely, safe transitions to the next level of care.

Qualifications
