An experienced RN Case Manager for Mass General Brigham’s Home Hospital program supporting hospital-level care delivered in patients’ homes. The role works within an interdisciplinary Home Care team and partners with payers, patients, families, and community organizations to support safe transitions and utilization management. It is primarily remote and suited to an RN who values autonomy and clinical leadership in an innovative care model.
