The RN Case Manager, Hospice provides compassionate, patient‑centered hospice care to a caseload of 10–15 geriatric patients in the Northeast Twin Cities metro. Working a predictable Monday‑Friday schedule with limited on‑call duties, the RN coordinates interdisciplinary care, conducts assessments, and supports patients and families through end‑of‑life care.
