RN Case Manager I Chronic Care

A DaVita Integrated Kidney Care (IKC) Registered Nurse provides integrated care coordination for patients with late-stage chronic kidney disease (CKD) and end stage renal disease (ESRD) across the Philadelphia region. The role supports DaVita’s integrated care model, working autonomously and collaboratively with healthcare teams to improve patient outcomes and lower total cost of care. The position may be telephonic, field-based, or a hybrid of both and requires a home office and travel to dialysis centers as needed.
