About this role

The Outpatient Case Manager on the Population Health Management team provides intensive, comprehensive case management for patients with complex, chronic, or catastrophic needs. The role manages patients on an outpatient basis and during acute hospital episodes to ensure appropriate level of care, safe transitions, reduced readmissions, and connection to community and clinical resources. The Case Manager advocates for members and families, coordinating multidisciplinary care and facilitating patient education and self-management.
