About this role
The RN Case Manager oversees appropriate levels of clinical care for hospitalized patients and partners with social work on discharge planning, with emphasis on medically complex discharges. The role is assigned to specific hospital units and supports continuity of care across inpatient and community settings. This position is part time / per diem and participates in departmental orientation and cross-training.
