About this role
A Case Manager provides care coordination services within an interdisciplinary team to evaluate options and services required to meet individuals' health care needs and promote cost-effective, quality outcomes. The role integrates complex case management, utilization management, quality management, discharge planning, and coordination of post-hospital care while serving as a consultant and patient advocate. The Case Manager leverages clinical nursing experience to support assessment, planning, implementation, and evaluation of patient-centered care.
