About this role
Cleveland Clinic South Pointe Hospital is hiring an RN Case Manager in Warrensville Heights to support patient transitions across the continuum of care. The role partners with inter-professional teams to coordinate individualized transitional care plans and empower patients and families. This is a day-shift position with limited weekend rotation and an annual holiday requirement.
