About this role
Cleveland Clinic Lutheran Hospital is hiring an RN Case Manager to support patients through transitions across the continuum of care. The role partners with interprofessional teams to coordinate individualized transitional care plans and help patients and families self-manage. This is a day-shift position based in Cleveland at a Magnet-designated hospital.
