The RN Case Manager is a field-based, hybrid nursing role that coordinates and facilitates integrated care for patients with end-stage renal disease (ESRD) and chronic kidney disease (CKD) within an assigned territory around Fresno, CA. The position supports DaVita Integrated Kidney Care’s mission to improve patient outcomes, experience, and cost of care by working autonomously and collaboratively with patients, families, physicians, and interdisciplinary teams. This role requires travel 3-4 days per week and a dedicated home office.
