Registered Nurse Case Manager with DaVita Integrated Kidney Care providing integrated care for patients with late-stage chronic kidney disease (CKD) and end-stage renal disease (ESRD). This hybrid, field-based role is territory-focused (Springfield, IL area) and works autonomously and collaboratively to coordinate care between patients, families, providers and care teams to improve outcomes and reduce fragmentation.
