About this role
DaVita Integrated Kidney Care is hiring an RN Case Manager to support integrated care for patients with late-stage chronic kidney disease (CKD) and end-stage renal disease (ESRD). The role is field-based and hybrid, requiring travel 3-4 days per week within an assigned territory and a dedicated home office. The RN will work autonomously and collaboratively across multidisciplinary teams to advance DaVita’s integrated care model.
