The Southcentral Foundation RN Case Manager provides timely, appropriate care to customer-owners, focusing on quality and cost-effective health outcomes within case management and Advance Access standards. The role works a Monday‑Friday 8:00 am‑5:00 pm schedule with a 60‑minute lunch.
