RN Case Manager

The RN Case Manager provides highly complex professional nursing care and case management for individuals with intellectual and developmental disabilities at the State Supported Living Center. This role involves conducting comprehensive health assessments, developing and evaluating nursing care plans, and coordinating interdisciplinary team efforts to ensure continuity of care and quality service. The position requires leadership, training, and technical assistance to nursing and direct care staff while maintaining compliance with state and federal regulations.
