RN Case Manager

The RN Case Manager coordinates and delivers high-quality, patient-centered end-of-life care within the Community Health hospice program. This role involves assessing patient needs, developing individualized care plans, and serving as the primary clinical liaison between patients, families, and the interdisciplinary team.
