The Health Coach at Nebraska Methodist Health System is an RN-based role in the Fremont Family/Internal Medicine clinic that supports care transitions, patient engagement, and social support across the care continuum. The role identifies high-risk patients, coordinates referrals and care plans with providers and community resources, and supports quality and improvement initiatives. This is a full-time, clinic-based position working Monday through Friday.
