RN Embedded Care Liaison

The RN Embedded Care Liaison manages and coordinates patient care within an assigned hospital, ensuring a seamless and high-quality patient experience aligned with organizational goals. This onsite role conducts bedside visits to assess needs, identify discharge barriers, and facilitate communication across providers to ensure medically appropriate care. The position also drives initiatives to reduce medically inappropriate readmissions and post-acute placements by partnering with hospitals and PCP offices to build safe discharge plans.
