RN Escalation Nurse

The RN Escalation Nurse serves as the senior clinical escalation resource for chronic care management and remote patient monitoring patient populations, assessing complex or urgent concerns escalated from various teams and device alerts. This full-time remote role involves conducting direct patient outreach, determining appropriate interventions, and coordinating with providers to resolve escalated clinical issues. The position requires strong independent clinical judgment and the ability to maintain composure during high-acuity situations while providing oversight to the LPN Escalation team.
