RN Home Care Coordinator

The RN Home Care Coordinator serves as a member of the Interdisciplinary Team to manage provider orders, conduct phone triage, and oversee the home care nursing schedule. This role involves presenting nursing interventions for care planning, performing chart audits, and ensuring accurate documentation while supporting home care nurses with procedures and task organization. The position also includes administering prescribed medications and treatments during home visits and educating participants and their families on care plans.
