About this role
The Hospice RN Case Manager Weekend delivers skilled hospice nursing and case management to patients and families in home settings, supporting quality of life and symptom management. The role performs nursing assessments, documents care, and collaborates with the interdisciplinary hospice team while operating within Mass General Brigham/Cooley Dickinson VNA & Hospice policies. This position is weekend-focused and reports to the Hospice Manager.
