About this role
Hospice Nurse for SSM Health providing home-based hospice and palliative care across Sauk County and surrounding areas, visiting patients in homes, community residential facilities, assisted living and hospitals. The role supports interdisciplinary care planning under the direction of the attending physician and serves a patient and family-centered end-of-life population. Position is part-time (32 hours/week) with evening/day shift patterns and eligible for a $10,000 sign-on bonus for qualified hires.
