The RN Pt Navigator provides inpatient and outpatient complex care coordination for the Neurology clinic patient population, serving as the primary point of contact for patients and caregivers. The role includes extensive patient education, monitoring plan of care adherence, managing acute needs, and coordinating with specialists and primary providers to remove barriers and improve outcomes.
