RPN - Hospital Partnerships Program (Toronto Central Region)

A Registered Practical Nurse (RPN) with VHA Home HealthCare delivers home-based nursing services as part of the Hospital Partnerships Program in the Eastern region. This is a part-time visiting position (13:00–21:00) where clinicians work in clients' homes, schools and community settings and often manage their own schedules. The role is within a large not-for-profit home care organization that emphasizes client- and family-centred care and provides orientation, training, and mentorship. A valid Ontario driver’s license and reliable vehicle are required for travel between visits.
