TaskForce Prevention and Community Services
The Ryan White Case Manager supports people living with HIV in accessing, engaging in, and remaining connected to medical care and supportive services. Through client-centered assessment, individualized care planning, service coordination, and advocacy, the role helps clients navigate barriers to care and strengthen stability and self-sufficiency. The position involves maintaining an assigned caseload, completing required assessments, and coordinating referrals across medical and supportive service providers.
