Scheduling Coordinator

This role is responsible for scheduling resources to outpatient primary care or specialty offices and accurately documenting information in the medical record and scheduling system. The position involves coordinating care across the Medical Group, following up on referrals, and working collaboratively with nursing coordinators to ensure a continuum of care. Success is measured by the number of appointments scheduled directly from the ED, Urgent Care, or other designated locations prior to patient discharge.
