About this role

The Clinical Pharmacist at Medical Home Network serves as a patient-facing provider and clinical resource to care teams within Federally Qualified Health Centers. The pharmacist is embedded in partner practices to enhance medication safety and strengthen performance in value-based care models such as Medicare Advantage Stars. This is a remote, part-time role at approximately 16 hours per week.
