Service Coord - RN Valley

Service Coordinator RN manages member caseloads to maximize health, wellbeing, and independence by assessing needs and coordinating Long-Term Services and Supports within the community. The role involves collaborating with physicians, families, and multidisciplinary teams to develop person-centered care plans and ensure timely, cost-effective interventions for chronic conditions. RNs, Nurse Practitioners, or Physician's Assistants may fill this position, focusing on pediatric and managed care populations including STAR+Plus members.
