Service Coordinator

The Service Coordinator develops, implements, and monitors services for individuals with intellectual or developmental disabilities under the direction of the Service Coordination Program Manager. This role involves facilitating person-centered planning, coordinating with various service providers, and conducting regular home and community visits to ensure effective service delivery. The position requires strong advocacy, program coordination, and administrative skills to support the assigned caseload.
