Service Coordinator

This role supports families with children from birth to three who have developmental delays or medical conditions, conducting home visits and virtual meetings to provide education and support. The position involves creating and implementing Individualized Family Service Plans (IFSPs) while coordinating referrals, evaluations, and advocacy services. The coordinator manages client records and collaborates with clinical supervisors and community providers to ensure comprehensive care.
