Service Coordinator

This role involves maintaining a caseload of families and functioning as a member of the child's interdisciplinary team to develop and monitor Individualized Family Service Plans (IFSPs). The position requires supporting families by connecting them with community resources, coordinating transitions to Part B services, and providing advocacy and crisis support. The Service Coordinator ensures accurate documentation and confidentiality while collaborating with local school districts and other agencies to meet the needs of infants and toddlers with disabilities.
