Short Breaks Link Workers

Nottingham City Council is seeking Short Breaks Link Workers to support children and young people with disabilities and complex health needs in accessing their community and developing independence. The role involves planning and participating in person-centred activities on a 1:1 or 2:1 basis, both in the young person's home and within the community. Workers will help young people achieve their aspirations while providing respite for families and full-time carers.
