Short Breaks Link Workers

The Short Breaks Link Worker role supports children and young people with disabilities and complex health needs in accessing their community, developing friendships, and gaining independence. The position involves planning and delivering person-centred activities on a 1:1 or 2:1 basis to help young people achieve their aspirations and improve their quality of life. This community-based role requires flexibility to work weekends and evenings while ensuring the dignity and safety of the individuals supported.
