Short Breaks Link Workers

Nottingham City Council is seeking Short Breaks Link Workers to support children and young people with disabilities and complex health needs in accessing their community and developing independence. The role involves planning and facilitating person-centred activities on a 1:1 or 2:1 basis, both in the young person's home and within the local community. This position requires a compassionate and proactive individual capable of managing challenging behaviours and adapting to diverse communication needs.
