Short Breaks Link Workers

Short Breaks Link Workers support children and young people with disabilities and complex health needs to access their community, develop friendships, and gain independence. The role involves planning and delivering person-centred activities on a 1:1 or 2:1 basis to help young people achieve their aspirations and improve their quality of life. This community-based position requires flexibility to work weekends and evenings while ensuring the dignity and safety of the individuals supported.
