Social Care Advisor

This part-time, permanent role serves as the first point of contact within a multi-disciplinary team, delivering person-centred interventions to support vulnerable adults in Hull. The position focuses on enabling individuals to shape their own lives, identify outcomes, and sustain independence through solution-focused conversations and access to community resources.
