Social Care Network (SCN) Program Manager

The Social Care Network (SCN) Program Manager oversees the implementation, coordination, and day-to-day operations of the Ali Forney Center's participation in New York State's Medicaid 1115 Waiver Social Care Network initiative. This role ensures staff compliance with state and managed care requirements while serving as the internal operational lead for screening, navigation, and service delivery across multiple boroughs. The position involves supervising staff, developing workflows, and collaborating with finance and external partners to maintain high-quality service delivery and reimbursement readiness.
